We are embarking on a marketing campaign of large scale. We believe this will bring much attention and new interest to Tera-Mai™ very shortly. As we are certain that you will benefit from our exposure, and due to the fact that our marketing costs have significantly increased we are certain that your small investment will bring great rewards.

IN SIGNING THIS FORM I AM AGREEING TO COMPLY WITH ALL TERMS OF THE TERA-MAI TRADEMARK POLICY AS STATED BELOW AND I HAVE INCLUDED MY LINEAGE (Tera-Mai™ Seichem and/or Tera-Mai™ Reiki) ON THE BACK OF THIS FORM THAT GOES BACK TO THE HOLDER OF THE TERA-MAI™ TRADEMARK, KATHLEEN MILNER.

I am also enclosing a $100.00 registration or re-registration fee.   (Changes or additions are $25.00)

No fee for anyone changing their mind and wanting to have his/her name removed.

1. The only Reiki and Seichem initiations I am giving and/or receiving are the Tera-Mai™ Seichem and/or Tera-Mai™ Reiki initiations as they are clearly demonstrated and explained in the videos Tera-Mai™ Reiki Mastership and Tera-Mai™ Seichem Mastership, which were produced in the summer of 1998.

2. I am refraining from giving and receiving manmade attunements - I understand that it is left up to me to discern manmade from Universal attunements. However, I understand that manmade attunements will adversely affect the Tera-Mai™ energy.

3.    If I wish to do other Reiki and/or Seichem initiations, I will request that my name be removed from the Tera-Mai™ Registry and I will stop using the trademark Tera-Mai. I understand that my right to use this trademark may be canceled at any time upon written notice. If at a later date I wish to exclusively initiate Tera-Mai™ Seichem and/or Tera-Mai™ Reiki initiations, I may be reinitiated on a donations-are-accepted basis, re-register and pay the $100 fee.

4.    My instructor was a fully initiated Tera-Mai™ Seichem and/or Tera-Mai™ Reiki Master who was exclusively initiating and teaching Tera-Mai™ Seichem and/or Tera-Mai™ Reiki initiations and so on back to the holder of the Tera-Mai™ Trademark, Kathleen Milner.
